| dc.description.abstract | Background: The idea of fluid resuscitation using balanced crystalloid fluid in the form of ringer acetate is relatively new, some studies have shown a beneficial effect on the liver and microcirculation. The purpose of this study was to compare improvement of lactate clearance and base deficit when using ringer lactate versus ringer acetate. Methods: This study is a double blinded randomized controlled study to compare the result of fluid resuscitation using ringer lactate and ringer acetate in blunt abdominal trauma patients with second and third degree hemorrhagic shock. Lactate and base deficit assessments were performed in these patients at beginning and two hours after resuscitation. The results of the two groups then compared and analyzed using the Chi square test. Results: There were 33 patients included in this study. Divided into two groups, the first group of 15 patients were given ringer lactate and the second group of 16 patients were given ringer acetate resuscitation. In the first group, high lactate clearance with values> 20% were shown in 6 patients (31.6%) and in the second group there were 13 patients (68.4%). Patient with normal base deficit value, were shown in 6 patients of ringer lactate group (33.3%) and 12 patients of ringer acetate group (66.7%). Resuscitation using ringer acetate fluid significantly increased lactate clearance (P <0.018) and reduced base deficit (P <0.048). Conclusion: Resuscitation using ringer acetate increases lactate clearance and decreases base deficit in blunt abdominal trauma patients with hemorrhagic shock compared with ringer lactate. | |
