ABSTRACT
Background and Objectives : Hypertension is considered as a risk factor for heart failure,
Based on heart failure risk calculation available score, only systolic blood pressure, smoking left ventricular hypertrophy, and comorbidities are included. Also, study that observe the characteristics of heart failure patients with hypertension are still a few and have not been found in Indonesia. In addition, there are other characteristics that are thought to be related, namely the duration of hypertension, hypertension in pregnancy, and drug consumption. Thus, this is the first study that observe that three characteristics and confirm the relationship between hypertension and heart failure.
Methods : This research is a descriptive study using registry data of heart failure patients at RSUP Dr. Hasan Sadikin Bandung in the period February 2020 to September 2022 and the exclusion criteria were incomplete registry data. We observe 17 variables, starting from baseline characteristics, hypertension characteristics (hypertension in pregnancy for female patients and duration of hypertension), medications consumed, medical history, and clinical characteristics (hypertension status, NYHA class, and followed up for three months).
Results : a total of 389 heart failure patients, which 71 patients had hypertension and were included for analysis. Patients mean age 49.2 years ±12.5, 55% were female and mean BMI 25.5〖kg/m〗^2±4.08. There were still active smoker patients (39.4%). From female patients majority have hypertension in pregnancy (56%). Patients experience hypertension with mean duration for five years until developing heart failure and mostly have reduced ejection fraction (77%).
Several patients have comorbid, mostly patients have chronic kidney disease (94%). Based on echocardiography, mostly patients have left ventricular hypertrophy (93%). Also, 11.4% patients have atrial fibrillation.
All patients already had antihypertensive drugs, the least one is CCB (8.5%). 39.4% patients still have uncontrolled hypertension and mostly have NYHA functional class III (52.1%). After three months follow up for 71 patients, with the result four patients (3%) died.
Conclusions : The hypertensive patients develop heart failure for five years in average. Mostly patients have reduced ejection fraction and left ventricular hypertrophy. The female one, majority had a history of hypertension in pregnancy. Many patients still have uncontrolled hypertension.
